The Salem VA Medical Center is recruiting for a part time, permanent, Physician (Home Based Primary Care) in the Geriatrics & Extended Care Service Line located at the Salem VA Medical Center, Salem, Virginia. Actively engages in case finding, developing and maintaining Home Based Primary Care program education for the facility, quality improvement and administrative functions of the Home-Based Primary Care program. Acts as a liaison and primary care provider for the Home-Based Primary Care agencies. Covers a cap limit of 75 patients enrolled in the Home-Based Primary Care program and participates in HBPC PI meetings as well as Geriatrics and Extended Care staff meetings. Works with the Home-Based Primary Care team to develop optimal care for the veterans enrolled in the Home-Based Primary Care program. On a consultative basis, Home Based Primary Care patients will receive care evaluation and follow up of care planning initiatives. Assist with training students, residents and fellows in rotation for Geriatrics and Extended Care Service Line. Work Schedule: Monday through Friday, 7:45 AM to 4:30 PM
