Seeking an experienced physician to join the Providence Medical Group at Home team, where you will be providing home-based urgent and primary care for patients in a home setting. In this role, you will provide consultations for acute conditions and transitions of care, in addition to primary care for a limited group of complex, permanently home-bound patients, who are not best served in a PMG clinic setting. Work with an interdisciplinary team that includes physicians, APCs, pharmacy, social workers, nurses, community paramedics, and a chaplain. • Full-time preferred, part-time negotiable
• Monday to Friday, daytime hours
• Outpatient primary care in a home-based environment including telehealth visits
• Board certified Family or Internal Medicine physicians
• Ideal candidates will have previous clinical experience working in Primary Care, Hospitals, Home Health, Geriatrics and/or Palliative Medicine
